Queensland Numbered Acts Queensland Numbered Acts(1) An approved service provider may apply to the chief executive for cancellation of its approval as an approved service provider.
(2) The application must be in the approved form.
(3) The service provider must also provide any other relevant information reasonably required by the chief executive to decide the application.
