additional fee equal to the cost to the hospital or health centre of
providing, or arranging for the provision of, that transportation

Subclause (2) does not apply to the transportation of a patient with a
retrieval team provided by a recognised hospital where a retrieval fee for the
provision of such a team by the hospital during transportation is applicable
under Schedule 1 or 2
