MEDICAL PRACTITIONERS REGULATIONS 1999 - REG 9

9—Application for registration by a company

An application by a company for registration on the general register must be lodged with the Board in the form set out in Form 3 of Schedule 1 and be accompanied by—

            (a)         the constitution of the company; and

            (b)         the appropriate registration fee set out in Schedule 5; and

            (c)         the appropriate annual practice fee set out in Schedule 5.
