For the purposes of section 22(9) of the Act, an application for compensation is to –

(a) be in writing; and

(b) be addressed to the Secretary; and

(c) specify –

(i) the name and address of the claimant; and

(ii) the name of the employer at the time of incapacity; and

(iii) the grounds for the claim for compensation.
