For Form 72K of the Principal Rules substitute —

"RULE 72.06(3)

FORM 72K

NOTICE BY EMPLOYER THAT JUDGMENT DEBTOR IS NOT IN EMPLOYER'S EMPLOY

To the Registrar,

And to the [ judgment creditor ] of [ address ].

An order was made on [ insert date ] that I make payments to the [ judgment creditor ] out of the earnings of the [ judgment debtor ].

I give notice that I am not the employer of the [ judgment debtor ] [ or I ceased to be the employer of the [ judgment debtor ] on [ insert date ].]
