A conciliator may
correct a conciliation decision or a certificate of outcome to the extent
necessary to rectify —
(a) a
clerical mistake; or
(b) an
error arising from an accidental slip or omission; or
(c) a
miscalculation of figures or a mistake in the description of any person, thing
or matter referred to in the decision or certificate; or
(d) a
defect of form.
